If not restrained it can jump out, if restrained it can hang over the side if trying to get out and bring the bike down. The dog is securely harnessed into the inside of the basket and cannot get out. Furthermore it is very easy to stop the bycycle very quickly if there is reason to. A motorists nightmare, but plenty of dogs in front baskets, and trailers ,kids in trailers and trailer bikes, good fun ,no problems. I have never had cause to carry a dog on a bike but one suggestion, having seen your opening photo, is that if you are going to use a front carrier/basket, is there anyway that it could be lowered (modified if necessary) to as close to the front wheel as possible?
