Three forms, as I found I needed a D2 as well as the application and medical forms. Came back on Friday, great. Included a paper licence as well, with instructions to destroy it on the 4th June (?) as it will no longer be legal. (If you have a plastic version) But..... I noticed it runs from when it was issued, rather than my birthday.
