AFAIK you will be supplied with a photocard (new type) licence.

 

If you apply physically (i.e. by post) you will need to supply a passport photo.

 

If you apply online (through Gov.uk) then I think they will happily take a digitised copy of a nominated passport photo from Government records, as long as it is reasonably up to date.

You will have to send in the old paper one to get a new one and you can only use your passport photo if it less than five years old so unfortunately that counts yours out.

 

I've been checking as I'm waiting for the renewal form for my photocard as it is very near 10 years old now.

It's not unknown for licence to be returned with some categories missing, so make a scan of old licence before you send if off, whilst they might claim to not accept scan as proof and will have a record of dates you passed test.
